Supportive Services

At People’s Center, we know that health is often influenced by non-medical factors. Our supportive services are available to all patients who wish to build a stronger foundation for overall health.




In order to ensure effective communication between patients and our staff, People’s Center provides qualified interpreters free of charge for those with limited English proficiency and for deaf or hard-of- hearing patients. Patients seeking interpreter services can notify our call center staff when scheduling an appointment, or can notify front desk staff for walk-in appointments.


Managing chronic diseases such as diabetes, hypertension and depression require a team effort. At People’s Center, we believe patients need to be at the center of their care in order to manage illnesses that require long term care. To help each patient improve their health and quality of life, he or she is assigned to a care team that helps them learn about how to better manage chronic illness and create an individualized, realistic and culturally inclusive plan of action. Our care management team includes a primary care provider, community health worker, nutritionist and pharmacist. Our medical social worker can connect patients with community resources for housing, emergency assistance, legal resources, disability assistance, home care services and other benefits from the county, State Medical Review Team (SMRT), insurance, citizenship, financial resources, Adult Rehabilitative Mental Health Services (ARHMS) and Metro Transit handicap services.


Patients participating in our Care Coordination program are assigned a care team to help them achieve optimum health and wellbeing. Care coordinators educate patients regarding their chronic disease, help them schedule and keep their appointments and help them navigate complicated healthcare and social service systems. Care Coordination Includes:

  • Developing individualized wellness plans to improve overall health.
  • Connecting patients to community and health resources.
  • Scheduling medical appointments, including referrals to specialists.
  • Assisting with financial concerns or paperwork.
  • Wellness education and coaching.

All patients are welcome to participate in our Care Coordination program!


We realize transportation to and from the clinic can be a challenge and expensive for some of our patients. People’s Center works with our patients' health insurance and medical transportation vendors to arrange transportation for all appointments. For patients that do not have insurance or their insurance does not cover medical transportation, People’s Center will provide free transportation to and from the clinic. People’s Center is able to offer FREE legal consultation including issues related to immigration, housing, and family needs. Legal assistance is offered by an experienced attorney at the Cedar-Riverside Clinic on the 2nd and 4th Mondays of every month from 1 – 5 p.m. To schedule an appointment, please call 612-332-4973 or you can sign up at the front desk on the main floor.
