At People’s Center, we know that health is often influenced by non-medical factors. Our supportive services are available to all patients who wish to build a stronger foundation for overall health.
In order to ensure effective communication between patients and our staff, People’s Center provides qualified interpreters free of charge for those with limited English proficiency and for deaf or hard-of- hearing patients. Patients seeking interpreter services can notify our call center staff when scheduling an appointment, or can notify front desk staff for walk-in appointments.
Our medical social worker can connect patients with community resources for housing, emergency assistance, legal resources, SSDI, PCA, benefits from the county, State Medical Review Team (SMRT), insurance, citizenship, financial resources, Adult Rehabilitative Mental Health Services (ARHMS) and Metro Transit handicap services.
People’s Center is able to offer free assistance legal matters, including immigration issues such as permanent residency, citizenship, and refugee status. Legal assistance is offered by an immigration attorney at the Cedar-Riverside Clinic on the 2nd and 4th Mondays of every month from 1 – 5 p.m. Service is first come, first served and patients can sign up at the front desk on the main floor.
Patients participating in our Care Coordination program are assigned a care team which connects patients to community and healthcare resources, and helps patients navigate complicated healthcare and social service systems. Care Coordination Includes:
